The Bureau of Long Term Care is located in the Baltimore County Department of Health. Our mission is to provide and coordinate services for homebound individuals at risk of alternative placement and assist the uninsured in locating health services.

Description:
The Bureau of Long Term Care has 4 Divisions. The Division of Home Health Services provides skilled care(nursing, therapy, social work and aide) to homebound residents of Baltimore County under the physician's plan of care.
The Division of Patient Evaluation and Case Management is a group of programs that provide assessment,care planning,follow-up, and resource coordination for those at risk of institutionalization.
The Division of Community Medical Assistance Programs provides services to eligible Medical Assistance recipients. Services provided include personal care and transportation services.
The PartnersHIP program provides limited medical services to uninsured residents of Baltimore County via partnerships with other health care providers and community resource coordination.

History:
The Bureau of Long Term Care has been in the Health Department for many years. It's mission to serve the frail elderly in Baltimore County has expanded over the years to include service to disabled residents, in addition to residents without health insurance. All programs provide an assessment and assist with care planning. The Bureau is staffed with nurses, social workers, therapists, and support staff. We provide outreach services to groups on a variety of topics. We have close coordination with other county agencies and private agencies serving similar populations.
