OUR MISSION The passionate pursuit of excellence in nurturing the physical, spiritual, and emotional well-being of our patients and their families, while being responsible stewards of our human and financial resources. PHILOSOPHY VistaCare prides itself on providing care to all eligible adult patients and their families, regardless of the complexity of their medical needs or other patient and family issues. This includes anyone with a life-limiting illness and a life expectancy of six months or less.
Description:
VistaCare believes there should be no barriers to high quality end-of-life care, and we take pride in our Comprehensive Care philosophy. This means we accept and care for anyone who is eligible for hospice, regardless of the complexity of his or her medical needs. VistaCare patients retain control of their care and are encouraged to make choices. Our patients do not have to give up current treatment or surrender the hope that they may recover. There are no further conditions or restrictions, beyond eligibility requirements … allowing our caregivers to deliver exceptional care, without exception.
History:
VistaCare began in 1995 in Scottsdale, Arizona and now has 48 hospice care programs in 14 states.
