We assist clients to attain health care goals, while maintaining independence and dignity, through appropriate home health and hospice care and supportive services. We all promote wellness in the community utilizing our experience , knowledge and resources.

Description:
Home health and hospice staff and volunteers provide services to patients in the comfort of their homes. We are the oldest freestanding home health and hospice agency in Utah. We are Medicare and Medicaid certified and CHAP accredited. Staff includes nurses, therapists, social workers, pharmacists, certified nursing assistants, homemakers and volunteers.

History:
Founded in 1928. a United Way agency.
Developed first hospice program in the state. Have offices in Logan, Layton, Tooele, Provo, Price, and Salt Lake. Have a Flu vaccination program. Kaleidoscope Kids service children with life threatening circumstances.
