The Area Agency on Aging of Western Michigan (AAAWM) exists to help older adults and persons with disabilities live in their community with independence and dignity.
Description:
Funded with state, federal, and local dollars, AAAWM does the following: •plans, develops, oversees, and advocates for the in-home care of older adults, especially those with the greatest social and economic needs •contracts with providers to deliver personal care, assisted transportation, housekeeping, home chore, home delivered meal, respite care, outreach and assistance, and legal assistance services to older adults •provides Care Management services for older adults, in which a nurse and social worker team develop in-home care plans utilizing resources available in the community •administers the MIChoice Medicaid Waiver program which allows for in-home care of Medicaid eligible adults at risk of nursing home placement •serves older adults in Allegan, Ionia, Kent, Lake, Mason, Mecosta, Montcalm, Newaygo, and Osceola Counties •administers the Kent County senior millage
