Aseracare Hospice provides medical, emotional, and spiritual care to patients and their families as they face the end-of-life. Our services are provided in private homes, Nursing Homes, Assisted Living, and Community Based Retirement Homes. Our team supports and works with staff at facilities and caregivers in their homes. Eligible patients receive Medicare coverage at 100%.

Description:
We provide a team approach for care. Registered Nurses as Case Managers, Certified Nursing Assistants as Home Health Aids, Chaplains, Social Workers, a Dietician, and Volunteers comprise the team. Our additional comfort measures include: Prayer Shawls and Lap Robes, Music Memories, Pet Therapy and Pet Visitor Volunteers, a Celebration of Life Quilt, an Inspiration Library, and our Vigil Program at the end-of-life, called Tender Touch.

History:
Aseracare Hospice (Formerly Hospice Preferred Choice) has been servicing patients and their families, initially in Milwaukee County, since 1996. In 2001, we branched out to Ozaukee, Dodge, Jefferson, and now in Kenosha County. We have provided care at private homes and Kenosha Care Nursing Home for two years, and more recently at Washington Manor. All opportunities are in Kenosha County.
