Care Wisconsin is a private, nonprofit community organization dedicated to supporting adults in their choice to live independently. Founded in 1976, Care Wisconsin today offers long-term care services and care management programs on a regional basis.

Description:
In 1976, Elder Care was created by community leaders who sought to provide our elderly with an alternative to premature placement in nursing homes. Among them were the Madison Urban Ministry, Attic Angel Association, United Way and Bethel Lutheran Church. We started Madison's first Adult Day Center.

Thirty years later, we have stayed true to our mission of helping older adults live at home and in the community for as long as possible. Inspired by our vision to achieve leadership in providing community-based services, we have taken bold steps in national demonstration projects to benefit our community. We have gained national and international recognition in our ability to provide high quality health and long-term care with greater cost efficiency.

Programs:

Care Wisconsin Partnership
In 1995, Care Wisconsin partnered with the State of Wisconsin to be the first provider for the Wisconsin Partnership Program, a national demonstration program. Initial funding came from The Robert Wood Johnson Foundation. Care Wisconsin Partnership offers an opportunity for frail elderly to remain living independently at home.

Care Wisconsin Partnership improves the quality of health care and service delivery while containing costs. By reducing the use of expensive services like emergency rooms, hospitals and nursing homes, this program is cost effective. The model also reduces fragmentation in existing health care delivery systems.

Our provider network for this innovative program features 99 primary care physicians in Dane County and over 200 other providers including UW Hospital, Meriter Hospital, St. Marys Hospital and Stoughton Hospital.

Our two adult day centers provide health maintenance and socialization in inviting settings to support older adults with memory loss, functional decline, medical illness or isolation. This program also serves family caregivers - especially those who work -in their efforts to keep their loved ones living at home. For 18 years, Care Wisconsin has operated the only day center in Dane County dedicated exclusively to the specialized needs of older adults with memory loss conditions such as Alzheimer's disease. Specially trained staff strive to keep participants functioning at their highest possible level.

Our home care services program includes dedicated personal care workers and certified nursing assistants who help older adults with bathing, getting dressed, making meals, housekeeping, medication reminders, wound care, blood sugar checks and a range of other services.

Care Wisconsin InFocus Health Program
In 2006, InFocus was created for enrollees living permanently in nursing homes. This Medicare Special Needs program provides all Medicare health care, plus our Nurse Practitioner teams up with doctors and nursing home staff to identify and address health concerns early and maintain quality of life.
