The Center For Hospice & Palliative Care serves individuals with serious illness, and their families and caregivers through a continuum of care spanning the course of illness and bereavement. The Center's specially-trained professionals, support staff and volunteers assist individuals to make choices about their care, support those who are grieving and educate the professional and lay communities about end-of-life issues.

Description:
The Center For Hospice & Palliative Care is the parent agency for Hospice Buffalo, Home Care Buffalo, Life Transitions Center, Hospice Foundation, The Center For Excellence, Caring Hearts Home Care and CMI Education Institute. Together these agencies provide a comprehensive range of services that meet the needs of patients and families confronting serious illness or needing help with skilled home care, counseling or education related to grief, death and crisis.

History:
In 1974 the first Hospice in America was opened in Connecticut. Charlotte Shedd, Hospice buffalo founder, new of the good work that was being done in Connecticut and saw the need for these services in Buffalo. After several information seminars, many interested health care workers, and many volunteer hours, Hospice Buffalo became incorporated in 1976. This was the original organization which was the building block of what we now know as The Center For Hospice & Palliative Care.
