Our mission is to provide care and comfort to terminal patients and their loved ones within their home settings, while serving as an educational resource in the community on end-of-life issues.

Description:
Hospice care provides comfort care-physical, emotional, and spiritual-to patients who have an illness that is no longer responding to aggressive or curative treatment. Our goal is to enable patients to remain in their home setting and to be as comfortable as possible during the last stage of life. Hospice care is provided by a team of professionals including: nurses, home health aides, social workers, chaplains, licensed therapists, a medical director, children's bereavement coordinator, and specially trained volunteers. Our nurses are available 24 hours a day, seven days a week, to assist patients and their families with their needs.

History:
Established in 1979 and now part of Diakon Lutheran Social Ministries, Hospice Saint John is one of the oldest and most experienced hospices in the state of Pennsylvania. Specializing in comfort care, Hospice Saint John offers services to people of any age in Eastern Pennsylvania and is dedicated to providing dignity, comfort and peace to its patients.

 Travel west on Route 12. When it becomes 422W, take State Hill Road exit, turn right at the bottom of the ramp onto State Hill Road. At second traffic light, turn left onto Spring Street. Take first right onto Reed Avenue. Building sits down on the right; offices are second to the end.
