Community Health Connection supports access to health services for financially eligible, uninsured individuals that reside in Eliot, Kittery, York, Ogunquit, Wells, North Berwick, Berwick, South Berwick and Lebanon, Maine by collaborating with the local health care community.
Description:
Community Health Connection works to connect the uninsured in Southern York County with local physicians that provide free health care along with services donated through York Hospital. CHC also provides a Prescription Assistance Program (PAP) to connect patients to free or low cost medications available through pharmaceutical company programs. The most recent addition to the program is a Medication Care Coordinator to assist with medication education, compliance and reconciliation.
History:
Community Health Connection began serving the area through York Hospital in 1999 when the program started. Since that time over 900 people have received services and over $3,000,000 in services have been donated through the local physicians and York Hospital.
