Description:
ServiceLink is part of a statewide network of 10, community-based resources, with the common purpose of providing information and supportive referrals about resources for older adults, adults with disabilities, and families and caregivers. ServiceLink offers assistance through a toll-free telephone number, home visits, scheduled appointments, walk-ins and public education. ServiceLink provides information on such topics as: 1. prescription drugs 2. caregiver support 3. home care options 4. transportation 5. long term care planning and many other topics.

History:
In 1998,the NH Division of Elderly and Adult Services (DEAS)held public forums about New Hampshire's long term care plan, "Shaping Tomorrow's Futures". As a result of these forums, NH citizens expressed the need for community based offices that provide information and assistance to seniors, adults with disabilities and their families. The NH Legislature then enacted Senate Bill 409, mandating the establishment of a system of community-based focal points, that provide information and referral services to elderly and the chronically ill. Today, these focal points operate as the statewide ServiceLink network.
