Hospice of Michigan will be the premier provider of expert, compassionate care for people who are experiencing life threatening illnesses. Our mission is to ensure quality of life and a comfortable, peaceful death for all patients receiving
our care, and to provide support for their loved ones. We will serve everyone in our communities who needs and seeks our care and strive to improve the state of end-of-life care.
Description:
Hospice of Michigan assist people during the final stages of their lives. Hospice patients receive care from a dedicated team of professionals including physicians, nurses, certified home health aids, social workers, spiritual counselors, therepists, dieticians, volunteers and grief support counselors. Hospice aims to enhance the quality of life. Our goal is to control symptoms such as physical pain and to help deal with the emotional, social and spiritual issues that arise at the end of life. Hospice serves people of all ages, from all walks of life. They suffer from different illnesses, and have unique needs, but they all want to die with dignity, free from pain and surrounded by people and things that are meaningful to them. Hospice of Michigan volunteers are a dedicated group of men and women with many different interest and talents. They help in mnay ways: through direct contact with patients and families, grief support to bereaved families, administrative assistance and community relations.
History:
In 2002 two innovators in end-of-life care teamed up to provide quality hospice and home care to Washtenaw County residents. Individualized Hospice & Home Care teamed up with Hospice of Michigan and combined more than 20 years of excellence in care to southeastern Michigan. Individualized Care has been serving Washtenaw county since 1981 with it Ann Arbor based office. This affiliation between the two offers the latest advancements in pain and symptom control and palliative care medicine. Hospice of Michigan serves over 900 patients a day in 45 counties across Michigan.
