Description : Hospice assists patients with a life limiting illness and focuses on comfort rather than curative medical treatments. In Hospice, both the patient and the family are important participants. Care is provided in the patients place of residence-their home, a nursing facility, residential care facility or assisted living facility. The goal of Hospice is to make the patient as pain-free and comfortable while providing family member with the help they may need. Hours of Operation: Monday - Friday 8am - 5pm Office/ Hospice Nurses available on call 24/7
Eligibility Requirements: Hospice care is recommended when a physician certifies that a patient has a life expectancy of 6 months or less or when a patient desires no future curative treatment and wishes to return the comfort of home.
Intake Procedure: Call the Ambercare intake team at 505-556-8409 and a Hospice Team Professional will meet with you to determine if hospice is an appropriate service for you at this time.
Insurance Information: Medicare, Medicaid, and some private insurances will cover 100% of hospice services. If you qualify under one of these, it is likely that you will pay nothing for hospice services. Please call Ambercare Hospice if you have any questions about your insurance coverage.
