Description:
Care is provided by a team of professionals and volunteers. Under the direction of the patients physician and our Medical Director, the team follows a personalized plan to address physical, emotional, and spiritual care. Our highly trained and experienced staff includes nurse case managers, social workers, chaplains, certified nursing assistants, and therapists.

History:
Our agency is still very new. We started taking patients in June of 2006. It was established to service the growing need of end of life care in the Boise area.
