The Elders' Actualization Project

The mission of the Elders' Actualization Project is to recognize and support the potential for growth and development of all seniors and to develop on-the-ground, workable methods to integrate that concept into senior support services. The Elders' Actualization Project applies new research demonstrating that cognitive stimulation, personal growth and social engagement are basic human needs.

Description:
The Elders' Actualization Project is a non-profit program for at-risk, frail, socially isolated and home bound seniors residing in Richmond, San Pablo, El Sobrante and El Cerrito, California.

There are two programs. The first is Care Management, which provides assessment and care planning, ensuring that basic services are in place to prevent unnecessary institutionalization. Assessment includes creative, educational and community engagement interests and goals.

The second program is the Elders’ Learning Community.
The Learning Community is a self-directed and learner-centered model that follows key research recommendations in cognitive health through the use of humanistic educational approaches.

Working with volunteer Learning Partners, the Learning Community focuses on identifying and developing each senior participant’s interests and weaves together learning partnerships and peer-led small groups.

The interests and creativity of participants and Learning Partners animate the Elders’ Learning Community.

History:
The Elders' Actualization Project emerged in 2005 with Opportunity West acting as fiscal agent. The Elders' Actualization Project has delivered services for three years based on a paradigm shift to a more complete view of human potential for seniors.
