We are committed to advocacy for the needs of terminally ill persons and their families, while dedicating ourselves to provide comfort, dignity, privacy and care efficiently and effectively. Our commitment to the hospice philosophy of care includes a compassionate staff, pledged to improve and enhance the care we provide, respect the Patient's choice of continuity of care, respond to changing community needs and serve as a resource to others for improving the availability of quality hospice care.
Description:
Hospice affirms life. Hospice neither hastens nor postpones death. Hospice exists to provide support and care for persons in the last phases of incurable disease so that they may live as fully and comfortably as possible. Hospice recognizes dying as a normal process. Hospice exists in the hope and belief that, through appropriate care and the promotion of a caring community sensitive to each patient's individual needs, patients and their families may be free to attain a degree of mental and spiritual preparation for death that is satisfactory to them. Hospice of Nursing Placement provides care to patients in their homes and at nursing homes; which includes pain control, personal care, spiritual needs and clinical social work services to meet patient's social and emotional needs. Our specially trained hospice team includes: Director of Clinical Services, MD, Case Manager, RN, Spiritual Leader, Clinical Social Worker, Home Health Aides and Volunteers. Care is available to our patients 24 hours per day. Our team members are dedicated to providing each patient support to make decisions about how and where they want to spend the rest of their lives and assurance that their advance directives will be respected.
History:
Hospice of Nursing Placement was established in 1997. This expansion at Nursing Placement, Inc. (our parent organization established in 1979) was made to meet the needs of those patients receiving home care, who were later diagnosed with terminal illness and would benefit from hospice services.
