Program name: SMMWC, Inc. - Mentor Me Program
Description : Self Made Men Women and Children-Mentor Me Program to serve 75-100 endangered students at Creighton School. The proposed program seeks to assist these children in receiving support and guidance from an adult mentor to help them improve their academic performance and their interpersonal relationships with peers, teachers, other adults, and family members. In addition, this program seeks to reduce the dropout rates, juvenile delinquency, and involvement in and gang activity among the target population.
Hours of Operation: Monday-Friday / 8am - 5pm
Eligibility Requirements: Mentors must be 18 years or older, reside in the Phoenix metropolitan area and agree to a 12 month commitment to the mentee, have common interests with mentee. Mentors will be required to complete background screening according to Arizona state and Federal guidelines annually. Mentor screening procedures will include: a completed written application, a drivers record check and proof of current auto insurance, attending a half day mentor training, two personal references, a personal interview, and parent/guardian consent. Potential mentors who do not pass this initial background check or subsequent background checks will be screened out of the program. Mentors must demonstrate an earnest desire to participate in the program. Mentor matches can be either male adult to male youth or female adult to female youth. In special circumstances and with approval, it is acceptable for female mentors to be matched with male youth. However, male mentors will not be matched with female youth. Mentors will also be required to attend mentor orientation and on-going trainings.
Intake Procedure: Potential mentee's will be identified by the school staff (teachers, social worker, and/or guidance counselor) as meeting the at-risk criteria, the Program Director, Mentor Coordinator, and School Liaison will work together with school staff to enroll mentee's into the proposed mentoring program. Mentees will undergo a screening process to determine suitability for the proposed program which will entail: completing an application form, obtaining parental/guardian permission, participate in a short interview to identify needs, strengths and weaknesses, agree to a one-year commitment to the program and to receive ongoing support for participation in the program, agree to spending a minimum of ten hours a month with the mentor, agree to comply with all program policies, procedures, and guidelines, maintain weekly communication with the mentor, attend mentee trainings as required, and be willing to communicate regularly with the Program Director, Mentor Coordinator, School Liaison, and teachers to monitor mentoring activities and progress in the program.
