The goal is to improve the quality of a person's life through pain and symptom control, emotional and spiritual support. Our vision is to exceed the expectations of our patients and families in providing outstanding care, promoting autonomy resulting in improved quality of life. Our Acclaim Hospice Team is committed to providing comfort and dignity to our patients.
Description:
Acclaim Hospice and Palliative Care offers services that focus on pain management and symptom control. A person is eligible for Acclaim Hospice care when a physician certifies that there is a life limiting illness not responding to curative treatment or the patient and family no longer seeks curative treatment. The primary care physician, patient and family need to be in agreement with the hospice philosophy. Financial coverage is available through the Hospice Medicare benefit and many private insurance companies. No person shall be denied services regardless of ability to pay.
History:
Our office opened in January 2008. We are very excited to be able to offer our services to the entire Miami Valley. The hospitals and care centers in the area have made us feel very welcome and appreciated. We also have an office in Columbus Ohio. That office opened 1 1/2 years ago and they are well established in that area. Acclaim Hospice is located off of 675. Take the 725 exit to Centerville. Turn left onto 725 and then left again at the first street which is Washington Village Drive. We are located in the first building on the left. Go to the 3rd floor and exit to the right from the elevator. We are the second door on the left. |
