Description:
Heartland Hospice provides compassionate care to individuals and their families when a patient has a life-limiting illness of six months or less. A team approach to care is provided by nurses, spiritual coordinators, social workers, physicians, bereavement coordinators, volunteers and nursing assistants. Care is provided in Skilled Nursing Facilities, Assisted Living and Independent Living Facilities and in private homes in communities throughout Delaware. Heartland is dedicated to supporting people live life to its fullest.
