Description:
A patient with a life-limiting illness has the right to live his/her final days comfortably, to the fullest extent possible, and to experience a dignified and peaceful death. Hospice services are designed to support the patient and the family's decision for the patient to remain at home or in a homelike setting in the final stage of his/her life. The goal of Home Care Hospice is to provide palliative care that addresses the physical, psychological and spiritual dimensions of service for the terminally ill and an integrated whole.
History:
Home Care Hospice has over 7 years of experience in providing hospice care within our commmunity. HCH was founded by a Registered Nurse with a passion for hospice care. Our reputation of exceptional care and service began in Northeast Philadelphia and quickly spread throughout the 5 county region. Today, Home Care Hospice, Inc. is the largest provider of hospice care in the Philadelphia area. Medicare/Medicaid certified and JCAHO accredited, Home Care Hospice provides culturally sesitive care in three languages: English, Russian and Spanish. We are a member of the National Hospice & Palliative Care Organization, and accredited by the National Institute for Jewish Hospice.
