The mission of The Home Care Network is to promote and maintain the independence and well-being of individuals and families in the communities served by Jefferson Health System by bringing to the home a coordinated and comprehensive array of health services which meet the acute, palliative, and chronic care needs of those individuals.

Description:
Hospice is a complete program of health care and support for people whose illness no longer responds to aggressive, curing medical treatment. Some of these illnesses may include cancer, advanced heart, lung or kidney disease, alzheimer's, ALS and AIDS.

Our hospice staff is composed of a dedicated team of professionals and volunteers who provide many kinds of care. Services range from pain control to help with everyday tasks to spiritual and emotional support and counseling.

The Hospice Philosophy focuses on helping patients live their last days to the fullest. The team of nurses, social workers, trained hospice volunteers, home ehalth aides, chaplains and bereavement counselors provide sensitive and caring support for people in the final stages of a terminal illness.

History:
The Home Care Network is dedicated to enriching quality of life for patients whose independence and dignity can best be preserved by receiving health care services and products in the comfort and safety of their own home.

The Home Care Network is further dedicated to the pursuit of excellence as a leader in assuring access to home care services. By embracing cutting-edge issues, The Home Care Network strives to be recognized as the preferred home care resource for residents of communities served by the Jefferson Health System.

The complete services of The Home Care Network include: Extended Home Care, Home Health, Home Infusion Service, Hospice and Palliative Care, Rehab Equipment Services, Respiratory and Medical Equipment.

 From areas WEST of Route 476 (Wayne, Devon, Malvern etc.): Take Route 30 (Lancaster Avenue) East Turn LEFT onto Radnor-Chester Road, (Radnor Hotel is on the corner) Drive to Second Light: King of Prussia Road; Turn LEFT Drive to first Light: Matsonford Rd; Turn RIGHT Drive to Second Light: Centennial Drive (first light is also called Centennial Drive); Turn LEFT into driveway then turn left again. Our building is on the right. (See below for Parking information). From Schuylkill Expressway (Route 76): Exit at 476 South Get off at next exit, Route 30 St David/Villanova Make a left at the end of exit onto Route 30 West Drive to first light: Radnor-Chester Road; Turn RIGHT Drive to Second Light: King of Prussia Road; Turn LEFT Drive to first Light: Matsonford Rd; Turn RIGHT Drive to Second Light: Centennial Drive (first light is also called Centennial Drive); Turn LEFT into driveway then turn LEFT again. From Blue Route (I-476) North or South: Exit at Route 30 St. David/Villanova Turn LEFT at the light at end of the exit ramp onto Route 30 West Drive apprx 1/3 mile to Radnor-Chester Road; (Radnor Hotel on the corner) Turn RIGHT Drive to Second Light: King of Prussia Road; Turn LEFT Drive to first Light: Matsonford Rd; Turn RIGHT Drive to Second Light: Centennial Drive (first light is also called Centennial Drive); Turn LEFT into driveway then turn LEFT again. Parking and Entering Building: You may park in any of the unmarked spots in front of the building but they are usually taken; in that case, drive up to the various parking lot levels or in the garage to find a spot; Enter in the front of the building and take the elevators in lobby to the fourth floor. Enter through the double glass doors, proceed to the receptionist desk on the right.
