The mission of The Home Care Network is to promote and maintain the independence and well-being of individuals and families in the communities served by Jefferson Health System by bringing to the home a coordinated and comprehensive array of health services which meet the acute, palliative, and chronic care needs of those individuals.

Description:
Hospice is a complete program of health care and support for people whose illness no longer responds to aggressive, curing medical treatment. Some of these illnesses may include cancer, advanced heart, lung or kidney disease, alzheimer's, ALS and AIDS.

Our hospice staff is composed of a dedicated team of professionals and volunteers who provide many kinds of care. Services range from pain control to help with everyday tasks to spiritual and emotional support and counseling.

The Hospice Philosophy focuses on helping patients live their last days to the fullest. The team of nurses, social workers, trained hospice volunteers, home ehalth aides, chaplains and bereavement counselors provide sensitive and caring support for people in the final stages of a terminal illness.

History:
The Home Care Network is dedicated to enriching quality of life for patients whose independence and dignity can best be preserved by receiving health care services and products in the comfort and safety of their own home.

The Home Care Network is further dedicated to the pursuit of excellence as a leader in assuring access to home care services. By embracing cutting-edge issues, The Home Care Network strives to be recognized as the preferred home care resource for residents of communities served by the Jefferson Health System.

The complete services of The Home Care Network include: Extended Home Care, Home Health, Home Infusion Service, Hospice and Palliative Care, Rehab Equipment Services, Respiratory and Medical Equipment.
